Is it strictly just 3-4-5 that the slippage occurs? Take it somewhere you can open it up at 1&2 and see if it's the same. A slipping clutch, if it's the clutch will happen in all gears.
It could be that you need to replace your clutch. For my gen 2 that I bought used, it seems the previous owner rode the clutch a bit. I also completely unrestricted the ECU, and unlocked its true power. Haven't done a double diaphragm spring conversion as of yet. Just got it back from the shop yesterday after replacing the clutch and the slippage is completely fixed. Parts and labor isn't too expensive if you choose not to do it yourself.
Another possibility is your master cylinder is rusted and the piston is failing to return properly and sticking in the middle. More than likely it is one of these two scenarios.
Yet another possibility would be you have a major clog in the hydraulic clutch lines. Other than these scenarios I really can't think of anything else.
