I have had many occasion of being behind those who refuse to use the right hand lane. I usually just pass them on the right, and keep on going.

On the throttle is the way to do it. I've had people in cages decide to finally pull right, just as I was passing them on the right. I do it quick and try to have room in case things get bad fast.
