I think what you're seeing is actually a 'toothed' nut...I'm guessing that's used to set the preload on the steering stem bearings and then the upper triple goes on with the steering nut. Looks normal to me unless I am missing something.
 
There is a secondary step for the top triple but usually the top will sit on to the upper nut.
