Unscrewed it does not release. Just want to drop in a longer spacer, an get the sag up an inch.

If that's as much as it will come out and it turns freely then I would attach a mole wrench and pull upwards as you turn.

A pair of water pump pliers should remove the loosened cap. Be careful re-installing the cap, the threads are very fine. A careless installation attempt could easily cross-thread the downtube/plug.
